Checklist when applying for Carer's Allowance

The following checklist is also part of the application form for Carer's Allowance. You must complete the checklist to ensure that you have answered all the questions and enclosed any relevant documentation with your Carer's Allowance application form.

1. Have you answered all the questions in this form, including those for your spouse or partner?
2. Have you ticked all the relevant answer boxes?
3. Have you enclosed the following certificates with your application?
  • Your birth certificate (long version)
  • Your marriage certificate (if relevant)
  • Certificate of separation or divorce (if relevant)
  • If you are getting a payment from abroad enclose a recent advice slip from the office issuing the payment(s) from abroad. For example, DSS.
  • Confirmation of Domiciliary Care Allowance (if relevant)
  • Current payslip
  • Your P60 or a statement from accountant, if self employed
  • Statement(s) from financial institutions (bank, post office, credit union etc.)
  • Payslip or letter, if getting an occupational or private pension
4. Have you and the person you are caring for (Caree) signed the application form where requested
