Checklist when applying for a medical card

  • Ensure you have completed all the relevant parts of the form and don't forget to sign your application.
  • Remember to provide proof of your PPS Number and that of your family or other dependants if necessary.
  • Part 3 of the form requires you to provide proof of all income and assets declared.
  • Part 4 of the form requires evidence of all outgoings, including loans, rents, mortgages and other costs.
  • You must sign Part 5 of the form.
  • You must complete and sign Part 6a.
  • Has your doctor of choice (GP) completed and signed Part 6b?

Staff in your Local Health Office or health centre can answer any questions you may have about medical cards and can help you with filling in the application form.
