Checklist when applying for a GP Visit Card

  • Remember to complete all the relevant parts of the GP Visit Card application form and sign it before forwarding it to your Local Health Office.
  • Have you provided proof of your PPS Number and the numbers of any dependants?
  • Part 3 of the application form requires you to provide proof of all income and assets declared.
  • Part 4 of the application form requires you to provide proof of all outgoings, including rent, mortgages, childcare costs and travel expenses to work.
  • Part 5 of the application form must be signed.
  • Part 6a of the application form must be fully completed - don't forget to sign this.
  • Arrange for your doctor of choice (GP) to complete and sign Part 6b of the form.
